When dental pulp is infected, performance of root canal treatment is required. Initially, local anesthesia is applied and the tooth is opened; the contents of root channels is cleaned and their length has to be precisely determined.
A very effective way is to measure the length of root channels with an electronic device – apex locator, which shows objectively working progress in real time. Our office possesses also a digital radiographic device, which enables comparison of the measured working length.
When the exact root length is defined, root channel is cleaned mechanically and chemically and is formed in the way that is required for filling. Finally, channel filling agents are used to hermetically seal the root channel and using an inlay the lacking part of the tooth crown is restored. Modern methods of processing and the usage of modern agents for filling root channel ensure the lack of adverse effects such as strong obscuration and coloration of treated teeth.
Occasionally, an additional root treatment can be required if the previous treatment was unsuccessful (e.g. due to low accuracy of handling). In these cases, we speak about retreatment of a tooth. It is performed in the same sequence, however, requiring largely increased attention of the physician in the removal of filling of root channel and opening of a new channel if this was not done during the initial treatment. Often, the repeated introduction of medicines for overcoming infection and potentiation of healing process in the jaw bone around the root apex is required. Filling of root channel and inlay are following. It is important that healing of the bone around root (where granuloma or cyst was present) be followed by radiographic image 3-6 months later to confirm healing. It should be noticed that there exist especially refractory cases, in which root treatment is unable to achieve healing, and this requires surgical treatment – removal of granuloma and root apex. This surgery is called apical osteotomia and has good indications in frontal teeth.
The main cause of inflammatory diseases of gingiva is mineralized tooth plaque or the so-called tooth stone. The tooth stone cannot be removed with brush and paste, but only by dentist.
An ultrasound device for cleaning teeth is used for the removal of rude deposition of tooth stone. After cleaning with the ultrasound device manual cleaning of fine deposition is made. Even with the finest tips ultrasound cannot achieve the quality that can be achieved with fine manual tools.
After scaling performance of polishing is recommended. It can be done in two ways: (1) prophy jet polishing and (2) polishing with paste, rubber and brushes.
(1) Prophy jet polishing (AirFlow) is made by specific devices dispersing water under high pressure in combination with baking soda over the surface of your teeth. This cleans remainders of plaque and removes coating.
(2) Polishing with rubbers, brushes and polishing paste. A paste containing abrasives and fluorides is placed on rubbers, correspondingly to brushes. It also removes coating and coloration.
The two methods are effective and remove effectively coating and spots. Usage of different solutions for mouthwash based on chlorhexidine, aldehyde and others or other remedies inhibit formation of dental plaque after cleanup of dental stone.
Dental bridges and crowns are two classical ways to restore destructed or missing teeth.
Crowns are used to restore the tooth integrity. Bridges represent an assembly of crowns and a middle moulded body, which replaces a missing tooth. Generally, crowns have either metal – metal-ceramic, or non-metal construction – press ceramic or zirconium. The later are very esthetic and are maximally close to appearance of the natural tooth.
Description of dental treatment
The procedure requires attention and patience but is not painful for the patient. The process goes through the stages as follows:
First, panoramic radiographic image is ordered.
Then, a detailed examination of the patient's denture as well as allergic tests are made.
The teeth, on which a bridge or crown will be mounted, are filed down.
An impretion, which is sent to dental laboratory, is taken.
A temporary crown is placed on the tooth.
After prepared construction is provided by dental technician, it is tested on the patient's tooth and if it fits, it is cemented.
Duration of dental treatment
The period for manufacture of crowns is from 5 to 20 days. Time depends on material, construction and technology, which is chosen together with the patient. Placement of construction is made in one visit.
Removable prostheses are constructions that are removable. They are used in the cases, in which treatment by bridges is not possible. They are kept in mouth by the rest teeth, by means of supporting-retaining devices or by soft tissues around them. If all teeth are missing, they are kept only by soft tissues or implants.
Prostheses can be modelled, moulded (with metallic skeleton), silicone prostheses (valplast) and simple plate prostheses.
Modelled, moulded prostheses are made with metal construction, which is strongly reduced and enables comfort during meal and speech of the patient. They are unmovable and almost non-retaining food. They do not change in time. Their manufacture requires the presence of sufficiently stable teeth in the mouth, which will carry prosthesis. To attach them to teeth hooks, which are moulded together with metal skeleton or special supporting-retaining devices such as joints, telescopic crowns, traverses and milled surfaces, are used.
Silicone prostheses - Valplast
Highly esthetic and anti-allergic prostheses, which do not have metal hooks. They are called also soft and elastic prostheses. They are extremely comfortable, enabling bending. They are intended for cases with sufficient number of teeth. They absorb sputum and pigments, due to which they change color and smell in time. For this reason, they are temporary prostheses (for about 6 months to 1 year).) They are comfortable for the patient as a temporary prosthesis during expectation of healing process after extraction or implantation.
Simple (plate) prosthesis
This prosthesis is made by semi-transparent plastics, which has the color of mucosa. It has low price. It can be a total (in missing teeth) or partial prosthesis. Total prosthesis in upper jaw is fixed by vacuum, created with mucosa of palate, and the total prosthesis in low jaw – by the precise attachment to tongue and cheeks. It is used when no other type of prosthesis can be manufactured.
